TXA Administration ≠ Automatic Lead Trauma Hospital Transport

The Indications for TXA Administration are NOT the same as the FTTS Guidelines​

 

Indications for TXA are DIFFERENT for FTTS

Although both the Traumatic Hemorrhage Medical Directive and FTTS are utilized to treat patients who have suffered a traumatic injury, there are different indications to utilize each​

Please verify that a patient who meets the criteria for TXA administration under the Traumatic Hemorrhage Medical Directive meets the indication to transport to the LTH under the FTTS​

These indications will not always align

Example 1​

  • 22-year-old male patient involved in a single-vehicle MVC (Car vs. cement median on city road)​
  • Obvious trauma to the chest (abrasions, no deformity) ​
  • No intrusion, ejection or death of another occupant​
  • HR 125 | RR 24 | BP 95/60 | Sp02 96% | GCS 15​
  • Meets criteria for Administration of TXA​
  • Does NOT meet criteria for FTTS​
    • Does not meet Physiological criteria (Step 1)​
    • Does not meet Anatomical criteria (Step 2)​
    • Does not meet Mechanism of Injury (Step 3)​
    • Does not meet Special Criteria (Step 4)

Example 2​

  • 22-year-old male patient involved in a single-vehicle MVC (Car vs pole on city road)​
  • Obvious head trauma to Rt parietal region, no other noted injuries​
  • HR 109 | RR 24 | BP 95/60 | Sp02 96% | GCS 12​
  • Patient does not follow commands​
  • Meets criteria for FTTS​
    • Meets Physiological criteria (Step 1) for Not following commands​
  • Does NOT meet Indications/Conditions for TXA Administration​
    • Does not meet Hemodynamic instability Conditions​
    • (also) Contraindication for Isolated head injury​

Example 3​

  • 22 year-old male patient involved in a single-vehicle MVC (Car vs streetlight)​
  • Obvious head trauma to Rt parietal region as well as chest (abrasion/seat-belt sign) ​
  • HR 125 | RR 24 | BP 95/60 | Sp02 96% | GCS 12​
  • Patient does not follow commands​
  • Meets criteria for FTTS​
    • Meets Physiological criteria (Step 1) for Not following commands​
  • Meets Indications/Conditions for TXA Administration​
    • Meets Hemodynamic instability Conditions​
    • Not an isolated head injury

FTTS and TXA Indications​

  • Although patients may often meet the indications for both FTTS and TXA Administration, each has its own unique set of criteria and conditions​
    • However: They do not always align​

Bottom Line:​

Please verify that a patient who meets the criteria for TXA administration under the Traumatic Hemorrhage Medical Directive meets the indication to transport to the LTH under the FTTS​
