Cross Certification

Overview

As part of the movement to standardizing processes across Base Hospitals, SWORBHP will recognize the auxiliary directives that paramedics have obtained at their previous Base Hospital and will receive automatic certification if the requirements set forth in this process are met.

Process

Procedure

The following is the process that we follow with regards to Cross Certifications at SWORBHP.

The following is the procedure that we follow with regards to New Certifications at SWORBHP.

Step 1

Paramedic Service to inform SWORBHP of newly hired paramedic(s) via the Paramedic Portal of Ontario.

Step 2

Paramedic to make a request for cross-certification via the Cross Certification Form (fillable PDF).

Step 3

Paramedic to complete the following sections:

Part A: Paramedic Information

Part B: Release of Information Authorization

Step 4

Paramedic to return the signed form to [email protected]

Step 5

SWORBHP to send the form to the previous Base Hospital to complete the following sections:

Part C: Current Certification Information

Part D: Current Auxiliary Directives Certification

Part E: Base Hospital Confirmation

Step 6

Previous Base Hospital to return the completed form to [email protected]
