Question:

FTTS says for any step 4 to consider a patch to the BHP. I was under the impression that a BHP could not direct traffic. Can you please confirm? 

Answer:

You are correct that a Base Hospital Physician (BHP) cannot direct ambulance destination. However, in the setting of the Field Trauma Triage Standard (FTTS), the BHP may aid in determining the need for transport to the Lead Trauma Hospital (LTH) via Step 3 and Step 4: BLS PCS wording; if the patient meets any of the Special Criterial listed above AND the land transport time is estimated to be <30 min to an LTH* or regionally designated equivalent hospital, determine the need for patient transfer to the LTH or regionally designated equivalent hospital.  

In this setting the BHP is not directing or providing a medical order as to where the patient should be transported but aiding the paramedic in determining the need for patient transport to LTH, as is part of the provincial destination policy.  This can be thought of as a consult point whereby the BHP can assist in determining the need for a LTH, given the high false positive rate of patients that meet Step 3 and Step 4 criteria that do not require the services of a LTH and can be appropriately treated in a non-LTH.
