Question:
If a combined crew configuration of PCP and PCP expanded scope (IV) attend a VSA that will be transported and it is the PCP's turn to attend, can the PCP attend it en route to hospital or should the PCP expanded scope have to take over in case of a ROSC with need of bolus?
Answer:
Per the Division of Responsibility Policy, “The PCP with expanded scope and certified in the Intravenous and Fluid Therapy auxiliary medical directive MUST attend to any patient who has received, requires, or is anticipated to require an intervention or treatment requiring the expanded scope of the auxiliary medical directive.” Although it is possible that a patient in cardiac arrest may not have a ROSC and require expanded scope treatment, it is also possible that the patient have a ROSC and therefore would benefit from this scope. Although not all cardiac arrest calls are anticipated to have a ROSC, the best potential outcome for this patient would be to obtain ROSC, in which case the PCP-IV would be the preferred attendee. When in doubt, ask yourself, is there a chance this patient would benefit from my expanded skill-set? If the answer is yes, then consider attending.
